Smart Discharges in Children Over Five: Expanding a personalized public health approach to improving discharge care in resource-limited settings

This prospective cohort study intends to improve the health of children over five in East Africa by developing a predictive model that identifies children of highest risk of post-discharge death, then developing targeted interventions to improve post-discharge care.
